A waiver must be signed in person before any piercing service can be performed. Please bring the following to your appointment:

Valid photo ID (driver's license or state ID)

Under 18

Parent or legal guardian must be present with their photo ID, plus the minor's birth certificate

These requirements are in accordance with Georgia state piercing laws.
